Hello everyone! I'm been browsing the forums for a few hours trying to figure out an issue with my mother's truck.  Here's the story:

It's a 2002 Nissan Frontier, 2WD,automatic transmission with 100K miles on it. There is some serious vibration coming from the rear when one drives up 39 to 45mph. The vibration seems to be originating in the rear of the vehicle. When you surpass 50 MPH the vibration goes away.  My mother in her infinite wisdom hired some guy to fix the truck. $700 dollars later and two replaced U Joints and we still have the problem. Additionally, the truck shakes only on certain road surfaces.

What has been done thus far?
1. The rear tires were brought to the front.
2. I jacked the rear up and checked for any play in the tires to see if it is a bearing issue, there is NO play.
3. Two U-joints have been replaced.
4. The tires were rotated and balanced 12/09/10

Let's tackle this problem systematically. Where do I start?

Is it a two piece drive shaft?? If the front and rear u-
joints are not on the same "plane" this will cause your
concern. This happens when someone replaces the center
carrier bearing assembly and doesn't install in the correct
position.

I got the tires rotated and balanced. The drive shaft is a "one piece shaft." Looking for some more causes to this problem. Pictures below:

One piece drive shaft pictured below:

Other U Joint replaced below:

Driver side rear shock, it's really old. I can shake the entire bed of the truck easily by myself. Seems really "loose." Could this be a cause?

Shocks sound like a good place to start.

Mnay times the one piece driveshaft throws a welded on or glued on weight that was put there in production to balance the driveshaft.  Even the loss of a small weight will give the symptoms you describe.  Look closely at the shft to see if you can see a place where a small weight might have been attached. (No rust, less rust, etc).  If so, a good machine shop can rebalance the shaft.

This is a very overlooked but common cause of vibration at speed and can drive you nuts if you don't know to look for it.

The bad shock may amplify this.  Had it happen to a coworker with a Ford truck.  Drove him nuts until I told him to get the dealer to pull the shaft and see if it was balanced.  All I am asking him to do is to examine the shaft for a missing weight.  But I agree with you...he should put in new rear shocks.

The individual who initially "fixed" the truck, said he took the drive shaft to the dealership and they balanced it.  I'm going to learn how to put some shocks on this sometime this week and I'll let you all know the outcome.
