would the transmition mount cause a vibration through the truck at certain speeds. I had the tires balanced and new u-joints put in. vibration is in the floor and it does it between 45-60. Even when its in 2wd or 4wd. The trucks a 2001 frontier with 166,000 miles on it.

Is it a 2 piece drive shaft? If so, has the center carrier ever been replaced. I have seen guys install these and get the u-joints out of phase and cause a vibration. possible motor and/or trans mounts, ex hitting the frame or floor pan ???

Also, drive shaft could have thrown a small balance weight and now be out of balance. Very common problem with symptoms as you describe!
