i have seen some reports on people getting better gas mileage and some people saying they are getting worse. its hard to tell cause people said they were also on the throttle more now that the truck seems to drive much better now.
i have had the timing advance on my titan for over a a year and half and never had any problems. i still to this day run the regular 87 octane gas and it has held the timing advance the entire time. if your running 91 octance right now then your really missing out on the timing advance.
the biggest advantage of getting the timing advance done is the throttle response is much better then stock. its hard to describe but once you drive the truck after having it done it feels much quicker on the gas. if you have access to the Consult 3, which is required to do for 2008 or newer models right now, then you have nothing to loose to try it on your frontier.
