For the power cable, you can cut a small opening in a electrical boot on the firewall. Or if you have a automatic, there should be a plate covering the hole for a clutch. I just removed that, drilled a hole in it, and sealed it up using some RTV once I had the wire in place. The rest of the cords are all inside the cab. I removed my lower seat belt attachment bolt, cleaned any paint there, and hooked my ground to that. But I also have an extended cab.
As for the RCAs, you'll only need just the regular cable provided you have an aftermarket H/U that has RCA outputs. If you still have the stock unit, you'll need what they call a Line Out Converter. This attaches into the speaker wires Left and Right +/-, and makes it so you can use the RCA Cables.
