So I have a 2010 Nissan Frontier 4.0.  It is a lease and has 30,000 miles on it.  I just bought it with 28,000.  It does that rattle sound only on cold starts and it goes away after one second at the most.

I brought it to Nissan and they charged me $125 to diagnose it and said there's nothing wrong with it.  I don't even know if they followed my instructions about making sure it was a cold start. 

Anyway, it sounds like the general consensous is that this is the timing chain? 

I was going to buy the truck back after the lease, because I really like it, but now I am not sure.  Will this likely get worse over time?

Again, this is a very common sound on this engine. As
Boomer stated using a high quality filter and oil is
necessary. Worse case scenario, I have seen the plastic
portion of the primary timing chain tensioner shoe break,
which allows the plastic to slide down the guide, allowing
the plunger to extend too fair not tensioning the chain
properly. This will create a very loud, usually constant
rattle. This usually occurs on high mile engines(150K +).

It is the timing chain.  Should last only a few seconds at start-up, cold or hot, and go away once oil pressure builds up.
