Thanks boomer. I spent 8 hours on there last night documenting what's going on. I talked with Rhonda Calico and she is such a great help.
I dropped my truck off at Red Bank Nissan this morning (the second opinion shop).
The Foreman was there at 645, 15 minutes early to help me out. I described the problem, how to replicate and he immediately said it sounds like a throw out bearing.
He looked under my truck and saw the debris described. He said that my clutch is worn but not worn out. The problem to fix is the throw out bearing not the clutch but only a tear down will tell. (frontierguy said this too) We then went for a ride and found the noise.
The service manager called and said that they must do a tear down. Throwout bearing, flywheel, pressure plates are all covered but the clutch isn't and if it's on the repair list then I have to cover the cost of it.
I called Rhonda who said that she will authorized the repair and will comp any bill out of good will. This is great but now hurts my situation. I have one good faith repair (if they do the clutch) on file with Nissan. What if down the line I have something go bad that truly deserves good faith? I don't think that this situation deems a good faith claim but I'm not going to look a gift horse in the mouth.
Then I called Jerry Morrow at Cherry Hill Nissan, 856-667-8300, because someone told me they had him do their similar repair. He remembered the claim and they had replaced the throwout bearing, flywheel, clutch and pressure plates on a truck with 20k miles. All was done under warranty and they claimed that the pressure plates were adjusted wrong from the factory.
Why can't all dealers be as good as this? They replaced the components at no charge and the guy didn't have to go good faith.
Either way I'm happy that it will be repaired and I won't have to pay but due to the circumstances I don't really have any leverage to help others out. I told Rhonda of the issue and she will try and help but she can't help everyone. I understand that Nissan can't good faith everything, they need to make money, but this issue should be covered or more well known.
If you hear of someone with this problem and they have low miles it will be easier to get repaired under warranty. If I knew it was the clutch / trans at 4k you can bet your butt I'd be having this done the first time for free. If someone suspects this in their 6spd, a noise or rattle from the dash that is weird or hard to replicate and describe I urge you to tell them the following. Tell them to accelerate hard in 3rd / 4th and slip their clutch in and out about 1/2". If the noise is louder starts or stops then this is the throwout bearing. The sooner you get this to the dealer the better.

It's all in the way that the warranty clerk writes up the repair order, if the pressure plate is used as the primary failed part(pfp) and it caused the clutch disc to fail prematurely - end of story. Like I said, the first dealer is jerking you around. By the way, which dealer did you buy the truck from, the same one that is giving you the run around??? By the way, my previous truck(96 tacoma) w/ 156,000 miles still had the original clutch when Toyota gave me 10,450 for it due to the rusty frame recall. Was going to get another 'yota, but seeing I work for Nissan, figured I might better buy one! Also the "Yota shifted smoother than my Nissan ever will. Still having a hard time getting used to it, escpecially first shift in the A.M. 1-2, feels notchy!!!! Hope you make out ok.

Update- I dropped it off yesterday morning, they dropped the trans, did diagnosis and called the reports in. They got the parts today and the foreman put it all together.
Parts- Clutch Flywheel Throw out bearing Pressure Plates
All of this was done in record time. Kevin McMahon at Red Bank Nissan in NJ took great care of me. The foreman came in early and left late to get my truck delivered to me. It's now 6pm, he got off at 4.
The foreman put it into Nissan as a bad throwout bearing, it was clearly making the noise. As I said before, he told me the clutch is worn but not worn out. He also put in for a new clutch, flywheel and pressure plate because of the wear on the clutch. All of this was covered by Nissan under warranty and there was no need for the good faith.
The fact that they were going to drop the trans without charge to diagnose because the throw out bearing was clearly the culprit was great. I also had the peace of mind that if a part wasn't under warranty Rhonda was going have Nissan NA eat the bill. I lucked out in this situation and I can only hope the best for anyone else in this situation.
Thank you guys for helping me diagnose this problem and seeing me through it. Frontier guy, you were spot on with your diagnosis from hundreds of miles away through a keyboard. I'm scared to see what you can do with a vehicle that you can actually touch!

Glad you made out ok. That is the way it should have been handled from the beginning, not being jerked around by the first dealer. It's too bad Nissan doesn't step -up and penalize the first dealer in some way!

Some dealers are just money dealers. They'll do everything possible to make money. Sell junk cars, gouge on repairs, and just have the worst customer service. We have a dealer here that sells flood cars and such, though they claim otherwise. They have the worst business practice and break the law most of the time. I've been in their repair shop and that place is just a nightmare and a accident waiting to happen.
Then you have other dealers that will go out of their way to do everything possible for a customer. The original owner of the Nissan dealer was the nicest old man who knew that with the right customer service people would be loyal and always come back. The service dept there was the best. And it was always a great time taking my Maxima in for an oil change. The owner would always come out and talk to me about my car (and try to buy it from me). The service reps also knew where I worked and didn't hesitate to let me walk into the shop and bs with the mechanics. It's such a shame that the owner retired. His son sold the dealership to another local company, and they've just stripped it down to nothing now.
